So what happened here is fairly simple but we need a base level of context first. So fundamentally hockey is a game that is played on ice. Another important bit of context here is that ice is a naturally slippy surface. What is seen on the video attached is simply someone trying to move around on a slippy surface and they lost their balance resulting in a fall, and since the surface is so slippy it was impossible to recover in time so they slid halfway across his own defensive zone to take out his own goalie.
Completely normal stuff here.
